Care Coordinator

TheCareCoordinatorworkswithamulti-disciplinaryteamincludingamentalhealththerapist,ayouth or family peer specialist, therapeutic recreation specialists, and education and healthcare professionals to engage and support families. This position provides a collaborative forum to engage a variety of child-serving partnersandadvocatefor theneedsofindividualsandtheirfamiliestosupportthefamilyremaining together and to improve the safety and satisfaction in their lives. This role provides direction in the developmentandimplementation ofcross-systemcare plansthataredesigned tohelptheyouth andfamily avoid out-of-home placement, end multi-system involvement, and incorporate additional resources and supports as necessary.

Qualifications:

1. HighSchoolDiplomarequired,BAincounselingorrelatedfieldpreferred.
2. RegisteredAgencyAffiliatedCounselorCredentialorabilitytobecomeregisteredwithin60days required.
3. Comprehensiveknowledgeofcommunityresourcesandpublicsystems.
4. Minimumthreeyearsofworkingwithyouthandfamilies preferred.
5. Exceptionalcommunicationandinterpersonalskills.
6. CommunityHealthWorkercertificationpreferred.
7. Musthavestrongmultitaskingandorganizationalskills.
8. Abilitytoworkinafast-pacedenvironmentandmakeethicalandsafedecisions.
9. Abilitytomanageindividualstressandperformessentialjobfunctionsinasafeandprofessional manner.
10. Mustbeabletopassacriminalbackgroundcheck.
11. Mustbe21yearsor older.
12. Validstatedriver’slicense.

Responsibilities:

1. AdherestoWISeprogramtimelinesforcompletionofcarecoordinationtasks.
2. Participatesintheon-callrotationtoensurefamilieshaveaccesstocrisisservices24/7.Thismay include phone support and in-person response.
3. Providesdirectioninthedevelopmentandimplementationofcommunityresourceprogramsto achieve desired outcomes for children and families.
4. UnderstandsandactivelypromotesWISeprogramsthroughoutthecounty,identifyingwaysin which efforts aimed at improving outcomes for children and families can be coordinated.
5. Recognizesbarriersthatoccurinasystemofcareanddevelopandimplementpractices necessary to sustain system of care improvements.
6. AssessesopportunitiesandbarrierstoachieveWISeservicesacrossallsystems.
7. Developslongandshort-termgoalsandobjectivesforthedevelopmentandimplementationof agencies WISe program services across all systems of care.
8. Seeksopportunitiestoeffectivelycommunicatewiththepublicandprivatechildandfamilyserving agencies to share information about the program.
9. ServesasliaisonbetweenallchildandfamilyservingagenciesandtheWISetreatmentteam.
10. Coordinatescross-agencycommunicationandplanningasneeded.
11. Usescompanyvehicleswhentransportingclients/familiestoprogramactivities.
12. Completeallrequiredtraininganddemonstratescompetencyinskillsnecessaryforrole.
13. IsawareofandavailabletoallWISeservicesasneededforadditionalsupportinperiodsofcrisis.
14. Maintainsprofessionalismandunitywhencommunicatingwithclients,theirfamilies,andother representatives; preserves client confidentiality at all times.
15. AttendsandactivelyparticipatesasarepresentativeofExcelsioranditssubsidiariesinmeetings, both internal and external.
16. EffectivelyrepresentsExcelsiorinthecommunity,toincludebutnotlimitedtotheCityofSpokane, the State of Washington and other venues as attended.
17. Adherestoorganizationalethicalstandards,policyandprocedures,mission,vision,andvalues.
18. Thispositiondescriptionisnotintendedtobeall-inclusive,andtheemployeewillalsoperform other reasonably related business duties, across other Excelsior subsidiaries, as assigned.
