Job Summary

The Case Management & Support Specialist is responsible for providing ongoing support, guidance, and resources to graduates of Adult & Teen Challenge of Arkansas. This role ensures a smooth transition from the program into independent living, employment, and continued spiritual growth. The coordinator will develop individualized aftercare plans, facilitate connections with community resources, and maintain regular follow-ups with program graduates to promote long-term success. This position reports to the Chief of Staff.

Qualifications:

· Passion for faith-based addiction recovery and alignment with the mission of Adult & Teen Challenge.

· Bachelor’s degree in social work, counseling, ministry, or a related field preferred, but a minimum of a High School Diploma or GED may be accepted.

· Experience in case management, social services, or pastoral care preferred.

· Strong interpersonal and communication skills.

· Ability to work independently and manage multiple responsibilities.

· Familiarity with addiction recovery and discipleship programs is a plus.

· Proficiency in Microsoft Office and case management software.

· Must have and maintain a valid Driver’s License, maintain automobile insurance coverage and have access to an automobile.

· Willingness to travel locally for outreach and networking events.

  • Must be able to pass drug and nicotine test.

  • Communicate compassionately and effectively with clients, health care professionals, and the general public.
  • Understanding of medical terminology as it relates to mental health care and basic family practice care.
  • Effective documentation skills.
  • Able to lift up to 30 pounds.
  • Physically able to walk, stand, and stoop for several hours each day.
  • Employee travels by automobile and is exposed to changing weather conditions.

Responsibilities:

  • Serves as part of the client’s treatment team.
  • Develop and implement comprehensive aftercare plans for program graduates.
  • Conduct regular follow-ups via phone calls, meetings, and digital communication to assess progress and provide support.
  • Connect graduates with housing, employment opportunities, counseling, and mentorship programs.
  • Provide spiritual encouragement and accountability to ensure continued faith-based growth.
  • Collaborate with community partners, churches, and employers to establish resources for graduates.
  • Maintain accurate records of aftercare services and track graduate progress.
  • Organize periodic alumni events, workshops, and support groups.
  • Serve as a liaison between graduates and the Adult & Teen Challenge program for ongoing mentorship and re-engagement opportunities.
  • Assist in crisis intervention and provide referrals to additional support services as needed.
  • Other duties as assigned.
