Job Description: Clinical Case Manager


Job Duties and Responsibilities:

  • Provides Clinical Case Management for 4 geographically separated treatment centers
  • Coordinates the plan of care for Clients in residential treatment center (RTC), partial hospitalization (PHP), Intensive Outpatient, and Outpatient.
  • Member of a multidisciplinary team
  • Assist with client evaluations, treatment planning, and client progress
  • Participates in teams meeting--supervision, clinical, and leadership
  • Serves as a liaison between client, external medical practices/organizations, and family
  • Connects clients to community resources
  • Facilitate scheduling of needed medical/other client appointments
  • Manages discharge planning and Client connection with follow-up care
  • Performs audits of discharge planning to ensure compliance
  • Support client understanding of discharge recommendations
  • Coordinates ongoing client care upon discharge (medication management, therapeutic services, primary care, etc.)
  • Meets with clients/guardians weekly or as needed to assess/provide resources and continuum of care
  • Communicates with utilization review team for level of care, census updates, and continuum of care planning
  • Coordination with outside providers
  • Document, coordinate, and disseminate admit, transfer, and discharge information
  • Build community relations, networking, and contacts in the healthcare continuum
  • Assist with marketing and networking as needed.
  • Coordinates with I.O.P. and outpatient services upon discharge if applicable
  • Conducts initial and weekly follow-up with each client for case management purposes
  • Other duties designated by supervisor

Minimum Qualifications:

  • Bachelor’s Degree
  • LMSW preferred
  • 3 years clinical case management experience
  • 3-year experience working in a behavioral/ mental health related field
