Eliot is seeking a strong, experienced, licensed RN, Nurse Case Manager, to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness. The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes. |

| Schedule: Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall schedule |
| Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase |
