Role Overview
As a Nurse Practitioner with Ultra Health Providers, you will serve as a trusted clinician for residents in your assigned facility. You’ll manage acute and chronic conditions, provide preventive care, and partner closely with nursing staff and administrators to ensure exceptional patient outcomes.

Responsibilities

  • Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
  • Conduct admission assessments, physical exams, medication reviews, and depression screenings
  • Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
  • Counsel patients and family members on treatment options and changes in health status
  • Manage clinical documentation accurately and in compliance with facility and state standards
  • Qualifications

  • Active and unrestricted Nurse Practitioner license in Tennessee
  • Current national board certification (AANP or ANCC)
  • Minimum 1–2 years of experience in primary care, geriatric, or long-term care settings preferred
  • Strong clinical judgment and communication skills
  • Commitment to compassionate, patient-centered care
