PERM - Physical Therapist - Outpatient

Unit: PT

Unit Description: **PERM CANDIDATES ONLY** Candidates must have 1 year Outpatient PT experience; must have Physical Therapist license for Pennsylvania at time of submission; and current BLS. The Rehab Department offers comprehensive rehabilitation services to individuals of all ages and levels of functional performance. This hospital based unit offers a very large gym with a mixed caseload from pediatrics to geriatrics including orthopedic, sports, and neurological diagnoses The rehabilitation department provides preventative education along with quality therapy programs to enhance participation in activities of daily living in the areas of physical therapy, occupational therapy, speech/language pathology and cardiopulmonary rehab.

POSITION SUMMARY – The outpatient Physical Therapist provides quality physical therapy services including assessment, intervention, program planning and implementation, discharge planning and documentation, and communication between other disciplines. The outpatient Physical Therapist completes an initial evaluation for patients referred for services and prepares and implements a patient treatment plan according to the results; Reviews and revises a patient treatment plan responding to acute changes, goals, patient status and facility policy; Creates a therapeutic treatment area; Completes discharge summaries; Functions according to APTA Code of Ethics and Standards of Practice.
