• This person will be working directly with members to assess, plan, and coordinate care

• They will have a caseload of about 75 members!

• Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care

• Monday-Friday 8a-5p, Full time with medical benefits offered

• This is an immediate Right to Hire opening!

• Pay range $32/hr-$35/hr salary negotiated based on relevant experience

Qualifications

• RN

• 2+ years of clinical nursing experience in acute care setting
