Position Summary

The Team Assistant provides administrative and operational support to the interdisciplinary hospice team to ensure efficient daily operations and quality patient care. This role serves as the primary administrative resource for clinical staff by coordinating communications, maintaining patient records, processing documentation, and supporting office functions.

Essential Responsibilities

  • Answer and route incoming phone calls.
  • Welcome visitors and assist patients, families, and vendors.
  • Maintain patient medical records and electronic documentation.
  • Enter admissions, discharges, transfers, and other patient data into the EMR.
  • Prepare patient charts and clinical documentation.
  • Coordinate physician orders and obtain signatures.
  • Schedule meetings and prepare materials for Interdisciplinary Group (IDG) meetings.
  • Order and maintain office and medical supplies.
  • Coordinate durable medical equipment (DME) orders and pickups.
  • Assist with volunteer coordination and administrative support.
  • Maintain accurate census reports and patient tracking.
  • Provide administrative support to the Clinical Manager and hospice leadership.
  • Ensure compliance with Medicare, state, and company documentation requirements.
  • Perform general office duties including filing, scanning, copying, and data entry.

Qualifications

  • High school diploma or GED required.
  • Two or more years of administrative or clerical experience, preferably in healthcare or hospice.
  • Experience with Microsoft Office and electronic medical records (EMR).
  • Strong organizational, communication, and multitasking skills.
  • Ability to maintain confidentiality and work effectively with patients, families, and interdisciplinary team members.
  • Hospice or home health experience preferred but not always required.

Final pay is determined based on factors such as experience, qualifications, location, and business needs at the time of hire. Some roles may be eligible for additional compensation components consistent with Arc Hospice & Palliative Care practices.
