Care Partner / CNA

At Abe’s Garden, we are establishing a national, evidence-based model of residential and day/evening care programs for those living with Alzheimer’s disease and related dementias.

POSITION SUMMARY:
Provide assistance and support with personal care and daily living activities for residents. Assist with serving meals and leading activity groups.

ROLES AND RESPONSIBILITIES:

• Ensure the safety of all residents, including maintaining an environment that minimizes fall risks.
• Assist residents in managing activities of daily living including but not limited to, eating, toileting, selecting proper attire, grooming, continence, dressing, bathing, walking and transferring (such as moving from bed to wheelchair).
• Offer companionship accompanying residents to meals and activities, engaging them in meaningful conversations and creating a sense of security for all residents and their families.
• Provide written documentation of services given, as well as communication regarding high-risk behaviors and/or critical incidents.
• Accurately document required information in electronic health record within established time frames.
• Create a sense of security for residents and their families through demonstrating an exceptional level of compassion and care giving.
• Respond to all resident calls and emergencies as quickly as possible.
• Demonstrate an understanding of state requirements by complying with all regulations, as required by various agencies for each area worked.
• Lead activity and engagement groups as scheduled.
• Maintain regular and timely attendance.

Qualifications

REQUIREMENTS:

• High school diploma or GED preferred.
• CNA certification preferred.
• Experience as a caregiver in an assisted living or similar setting.
• Ability to relate well to people and demonstrate appropriate, compassionate care for seniors.

PHYSICAL DEMANDS / WORK ENVIRONMENT:

• Must be able to sit / stand for 8 hours.
• Walking, bending, light lifting up to 50 lbs.
