LPN (Licensed Practical Nurse)

At Abe’s Garden, we are establishing a national, evidence-based model of residential and day/evening care programs for those living with Alzheimer’s disease and related dementias. POSITION SUMMARY:
Provide personal care, assist in recreational activities, perform assessments, and give companionship to the residents of the Abe’s Garden.

ROLES AND RESPONSIBILITIES:
Direct and supervise the Care Partners working within Abe’s Garden, and provide training and guidance as needed.
Observe, record, and report resident behaviors, symptoms, or changes in resident’s record and to oncoming shift.
Perform physical assessments and obtain/maintain resident health histories.
Record resident medical information and vital signs, note changes, and pursue more specific investigation as needed.
Administer medications and other treatments to aid various conditions.
Keep written documentation of services provided, as well as communication about high risk behaviors and/or critical incidents.
Respond to and treat resident emergencies.
Educate residents and their families about their conditions while establishing trust and rapport with them.
Provide companionship and emotional support to residents and their families.
On an as-needed basis, assist residents with daily activities, including but not limited to bathing, grooming, toileting, eating, dressing and engagement activities.
Act as liaison between resident, family, physician and ancillary services
Maintain continuing education necessary for keeping required credentials.
Maintain regular and timely attendance.
