We have an immediate need for a full-time Patient Coordinator to join our Case Management team.
The Patient Coordinator plays a vital role in ensuring effective communication and support between our healthcare team and our patients.

Responsibilities

  • The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources.
  • Accompany patients to their benefit renewal appointments to advocate for the patient and the program.
  • Visit patient homes and update patient care.
  • Effectively communicate and provide information to patients, caregivers, and physicians.
  • Understand HIPAA and maintain confidentiality.
  • Display a warm and friendly attitude.
  • Minimum Qualifications

  • Strong work ethic and relationship building skills.
  • Self-motivated with the ability to manage time appropriately.
  • Strong communication skills inside of the office and outside.
  • Experience with MS Office products (Outlook, Word, Excel, etc.)
  • Minimum associate level education preferred. (High school diploma or equivalent is required.)
  • Minimum 1-3 year of experience in a Care Team or Patient Navigation role.
  • Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices.
  • Healthcare experience preferred.
