Case Manager

All American Detox Center is a looking to hire a Case Manager to join our great team!

As a Case Manager, you will play a pivotal role in helping individuals on their journey to recovery from substance abuse. You will work closely with clients, treatment teams, and external resources to develop and implement personalized treatment plans, provide emotional support, and ensure clients have access to the necessary services for their recovery.

Qualifications:

Valid credential to perform recovery related services (RADT, RAC, CADC, CATC)

Current driver license and reliable transportation

Completion of TB Screening, Pre-Employment Physical, and Drug Test

Previous experience working the substance abuse/mental health clients

Responsibilities:

- Conduct thorough assessments of clients to determine their unique needs and goals.

- Collaborate with treatment teams to create individualized treatment plans.

- Advocate for clients by helping them access healthcare, social services, and community resources.

- Coordinate and monitor the delivery of services to ensure comprehensive care.

- Provide crisis intervention and emotional support as needed.

- Educate clients and their families about addiction, recovery strategies, and relapse prevention.

- Maintain accurate and up-to-date client records in compliance with legal and ethical standards.

- Offer supportive counseling to help clients build coping skills and resilience.

- Make referrals to external agencies and community resources to address clients' broader needs.

- Collaborate with external partners to enhance the continuum of care.

- Stay current with industry best practices and participate in ongoing training and development.
