The Case Manager manages a high-volume mental health intake process, including scheduling appointments and ensuring clients meet admission criteria. This positionĀ playsĀ anĀ integralĀ partĀ in client-therapist matching and caseload management toĀ maintainĀ continuity of care.Ā 

Responsibilities:Ā Ā 

  • Facilitate a high-volume mental health intake process by enforcing no-show policies, and scheduling intake appointments to ensureĀ the efficientĀ onboarding of new clients.Ā 
  • Provides intake triage and coordination for all virtual intake appointments, ensuringĀ appropriate screeningĀ andĀ timelyĀ connection.Ā 
  • Conduct an average of 40 to 45 daily interactions with new clients,Ā demonstratingĀ excellent interpersonal and communication skills toĀ establishĀ rapport and guide individuals through the intake process.Ā 
  • Facilitate client-therapist matching after intake and manage caseload reassignments during therapist transitions, leaves, or departures to ensure continuous, seamless care.Ā 
  • Perform other duties assigned by the Supervisor and Leadership.Ā 

Qualifications

  • Bilingual; fluent in English and SpanishĀ 
  • High School Diploma or equivalentĀ required;Ā Associate DegreeĀ in a related field preferred.Ā 
  • One year of experience in a social service or mental health setting is preferred.Ā 
  • One year of experience using an EHR systemĀ is preferred.Ā 
  • AbilityĀ to manage multiple tasks and prioritize effectively in a busy work setting is essential.Ā 
  • Strong capability to follow established workflows and effectively multi-task throughout the day.Ā 
  • Attention to detail is essential.Ā 
  • Exceptional customer service skills, with the ability to communicate effectively and professionally with clients and staff.Ā 
  • Demonstrates the ability to de-escalate, practice self-care, andĀ maintainĀ professionalism while handling emotional or mentally challenging situations.Ā 
  • Strong ability to multi-task andĀ maintainĀ attention to detail in a dynamic work environment.Ā 
  • Works collaboratively within a team andĀ maintainsĀ a positive, supportive attitude.Ā 
  • Thrives in a fast-paced environment, adapting quickly to changing demands.Ā 
  • Proficient in Microsoft SharePoint, Office Suite, Outlook, Word, Excel, and PowerPoint.Ā 
  • Familiar with the MS Teams video conferencing platform.Ā 
  • Able to perform with versatility and tact in a mental health care setting that requires flexibility and adaptability.Ā 
  • Maintains a positive, friendly, and professional demeanor, with a "can-do" attitude.Ā

Additional Information

Salary: $21.00 - $23.00 per hour

Compensation commensurate with experience and qualifications.
