Position Summary:
The Case Worker provides case assistance services to participants in SHOPP's Older Adult Center (OAC) program, helping older adults access benefits, entitlements, services, and community resources. The role includes outreach, participant registration, application assistance, supportive counseling, referrals, follow-up, and accurate documentation. The Case Worker reports directly to the Program Director and may provide day-to-day supervision and guidance to students or interns, as assigned, in coordination with the Program Director.
Essential Functions and Responsibilities:

Role-Specific Responsibilities
- Conduct Telephone Reassurance Services.
- Screen participants for benefits, entitlements, and services.
- Assist participants with applications for needed benefits, entitlements, and services, and follow up to ensure services are obtained.
- Provide supportive counseling as needed within the scope of the position.
- Provide information and referrals to appropriate agencies and identify community resources for consultation and referral.
- Maintain accurate, well-written, and timely case files using the assigned database.
- Complete work logs and track performance targets and participant outcomes for statistical reports.
- Conduct outreach and presentations on benefits and entitlements for SHOPP participants and community partners.
- Recruit and register participants for case assistance services and SHOPP's OAC educational and recreational activities.
- Develop and maintain partnerships and linkages with community groups.
- When assigned, provide day-to-day supervision of students or interns, including orientation, appropriate task assignments, work review, and feedback.
- Guide assigned students in developing professional skills and following SHOPP policies, confidentiality requirements, and participant service expectations within the scope of their placement.
- Coordinate with the Program Director and educational partners, as appropriate, on student learning objectives, evaluation input, progress, and concerns.
- The Case Worker may be assigned to work at other SHOPP locations as needed to support staffing coverage and program needs.
- Perform other tasks as assigned by the Program Director/ Leadership.

- Complete accurate and timely documentation, reporting, and administrative tasks

- Demonstrate flexibility in supporting evolving program and organizational needs
- Complete other duties appropriate to the position as assigned by the Program Director or Leadership.
Qualifications:
Required:
- Bachelor’s degree in social work, or an associate degree with a minimum of two years of experience in a social service setting in the field of aging.
- Experience with community-based services for older adults.
- Knowledge of Microsoft Office applications and the ability to maintain accurate electronic records.
- Ability to screen participants for benefits and services and assist with applications, referrals, and follow-up.
- Effective verbal and written communication skills, including the ability to conduct outreach and presentations.
- Strong organizational skills and the ability to maintain case documentation, track outcomes, and manage follow-up responsibilities.
Preferred:
- Knowledge and experience with the older adult population and community-based services.
- Bilingual in English and Spanish.
- Proficient in computer systems, case documentation, and data entry.
- Familiarity with VIVE system preferred.
- Knowledge of benefits, entitlements, and community resources available to older adults.
