Community Outreach Coordinator - CMHD - Lewistown

One Health's Central Montana Health District team is looking for a Community Outreach Coordinator to join our team in Lewistown, MT.

Location: Home base is Lewistown, MT however, this position is part of the Central Montana Health District sector of which serves 5 surrounding counties.
Schedule:Full-time |Flexible Hours|Travel Required
Pay:$20.55 - $21.37/hr DOE

One Health is seeking aCommunity Outreach Coordinatorto promote health, connect individuals to resources, and strengthen rural communities through education and outreach. If you’re passionate about helping others and thrive in a collaborative environment, we want you on our team!

What You’ll Do:

• Conductoutreach to promote One Health services and programs.
• Assistindividuals in developing health/social management plans and goals.
• Designandimplementprevention campaigns and education programs.
• Documentencounters andmaintainaccurate electronic records.
• Buildrelationships with healthcare providers, agencies, and tribal entities.
• Participatein community events, meetings, and trainings.

• Strong communication and documentation skills
• Ability to manage multiple priorities independently
• Professionalism, empathy, and clear boundaries
  

Required:

• Education:Associate’s degree OR 4 years’ related experience
• Experience: One (1) year of experience in Community outreach. Demonstrates advanced knowledge in community outreach to complete essential duties.
• License:Valid Driver’s License
  
• Certification: BLS (Upon Hire)

Preferred:

• Bachelor’s degree in health promotion, public health, health administration, or related field
• Certified Application Counselor (CAC) for Affordable Care Act (ACA)
