No Major Violation (Conviction) in the past 36 months Major Violations Include but not limited to:

- Leaving the scene of an accident

- Reckless driving

- Any alcohol or drug violation

- Vehicular homicide or assault

- Participating in and unlawful speed contest

- Eluding or attempting to elude a police officer
