About the Role
  • Assist participants to access their community and attend appointments

  • Deliver clinical and personal care: manual handling, hoisting, continence, PEG feeding, medication administration

  • Support daily living: hygiene, meals, mobility, transport

  • Monitor health changes and follow clinical care plans

  • Maintain accurate case notes and incident reports

  • Collaborate with families, allied health and nursing teams

  • Ensure a safe, clean, and engaging home environment

About You
  • Minimum 3-6 months’ experience or Min Certificate III/IV in Disability, Individual Support, Nursing, or related fields.

  • Experience with high physical needs and clinical care

  • Confident in manual handling, medication administration, and hoist transfers

  • NDIS Worker Screening, First Aid, valid Driver’s Licence, and insured vehicle
