We are seeking a dedicated and compassionate individual to join our team as a Family Model Provider. This position is part of a community-based residential alternative service program designed to support seniors and adults with intellectual and developmental disabilities. Responsibilities
- Provide daily living assistance, supervision, and support based on each individual’s Person-Centered Support Plan (PCSP).
- Foster an inclusive and supportive home environment that promotes personal growth, safety, and independence.
- Encourage participation in employment, community activities, and social engagement opportunities.
- Monitor and report changes in health, behavior, or needs to the assigned case manager or supervisor.
- Support the implementation of individual goals, skill development, and personal interests.
- Maintain accurate and timely documentation in compliance with agency and state requirements.
- Ensure medication administration, health appointments, and household routines align with care plans and agency policy.
- Communicate effectively with agency staff, family members, and other providers to ensure coordinated care.
- Participate in ongoing training and professional development as required by Stanford Heights.
Qualifications
- High school diploma or equivalent required.
- Experience providing care or support to individuals with intellectual, developmental, or physical disabilities preferred.
- Ability to provide a stable, safe, and inclusive home environment.
- Strong communication, problem-solving, and interpersonal skills.
- Must meet all background check and home safety inspection requirements.
- Valid driver’s license, reliable transportation, and proof of insurance required.
- Must complete required training and maintain compliance with state and agency standards.
