Are you a compassionate and dedicated Registered Nurse looking to make a significant impact in patients' lives? AdvisaCare is seeking a Full-Time Home Health RN to provide top-quality skilled nursing care in the South East Grand Rapids area. Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing treatments and procedures as required.
• Maintain accurate and up-to-date patient documentation and records.
• Coordinate care and communicate effectively with patients, families, and interdisciplinary teams.
• Ensure compliance with all safety, infection control, and regulatory standards.
• Participate in patient education and advocate for patient needs.

Requirements

• Current and valid Registered Nurse license for the state of Michigan.
• Minimum of 1 year of nursing experience, preferably in home health care.
• Ability to work independently and manage a diverse patient caseload.
• Strong communication and interpersonal skills.
• Reliable transportation and valid driver's license.
• CPR certification.
• Ability to pass drug screening and background check.
