POSITION PURPOSE

The Health Care Assistant (HCA) provides frontline, hands-on support to community members, assisting with daily living, personal care, and overall well-being. Working under the guidance of a Registered Nurse, the HCA plays a critical role in supporting Elders, individuals with chronic conditions, and those requiring additional care to remain safely in their homes and connected to their community. This role is relationship-focused and community-based, requiring a compassionate, respectful, and culturally safe approach to care.


ACCOUNTABILITY

Personal Care & Daily Living Support

  • Assist clients with Activities of Daily Living (ADLs): bathing, grooming, dressing, toileting, and oral hygiene.
  • Support mobility, including transfers, walking, and use of mobility aids or mechanical lifts.
  • Assist with positioning and comfort to ensure safety and dignity.
  • Assist with meal preparation, feeding, hydration, and light housekeeping related to client care.

Clinical Support & Observation

  • Assist with basic clinical tasks under the supervision of the Community Care Nurse (RN).
  • Take and record vital signs as directed; provide medication reminders within scope.
  • Support implementation of care plans developed by the nursing team.
  • Monitor and report changes in clients' physical, emotional, or mental health promptly.

Emotional Support & Community Engagement

  • Provide companionship and emotional support to reduce isolation and promote well-being.
  • Build trusting relationships with clients and families.
  • Support clients in maintainingconnections to community, culture, and daily routines.
  • Participate in community-based programs and initiatives where appropriate.

Documentation, Compliance & Cultural Safety

  • Maintain clear, accurate, and timely documentation as required.
  • Deliver care in a culturally safe, respectful, and trauma-informed manner.
  • Support clients in remaining connected to their culture, traditions, and community.
  • Work collaboratively with the broader health and community services team.


EDUCATION & EXPERIENCE

Required:

  • Health Care Assistant Certificate or equivalent recognized training
  • Registration with the BC Care Aide & Community Health Worker Registry (or eligibility to register)
  • Valid First Aid/CPR certification
  • Valid Class 5 Driver's License and ability to travel within the community

Preferred:

  • Previous experience in home care, community care, or working with Elders
  • Experience working with Indigenous communities
