Home Health Speech-Language Pathologist (SLP) – Mid-Michigan Talent Pool

About the Role

Help patients regain their ability to communicate, swallow safely, and improve their quality of life from the comfort of home. As a Home Health Speech-Language Pathologist (SLP), you'll evaluate and treat speech, language, cognitive, communication, and swallowing disorders through individualized therapy plans designed to help patients achieve greater independence and reach their functional goals. Working under physician orders and the established plan of care, you'll deliver evidence-based therapy that supports meaningful recovery and enhances everyday living.

This role is ideal for a compassionate, patient-focused Speech-Language Pathologist who enjoys building relationships and making a lasting impact. You'll perform comprehensive evaluations, develop and implement individualized treatment plans, collaborate with physicians and the interdisciplinary care team, educate patients and caregivers, and maintain accurate clinical documentation that meets regulatory and reimbursement requirements. Through your clinical expertise and commitment to exceptional care, you'll help patients overcome communication and swallowing challenges while supporting Optimal Care's mission of delivering high-quality, patient-centered healthcare.

Location

• Office Location: 4760 Fashion Square Blvd, Saginaw, MI 48604
• Service Area Options:
  • North: Arenac / Gladwin / Clare
  • South: Isabella / Gratiot / Shiawassee / Saginaw / Bay City / Midland / Genesee
  • This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Monday to Friday, 8 a.m. to 5 p.m.

Required Qualifications

• A Master’s or Doctoral degree in Speech Language Pathology
• State Licensed as a Speech Language Pathologist
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

• Minimum 2 years' experience as a Speech Therapist
• Home Health experience

• Interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregivers
• Knowledge of medical terminology
• Able to demonstrate problem-solving and conflict resolution, organization, and attention to detail
• Strong verbal and written communication skills
• Ability to operate computer equipment, FAX, and photocopy machines
• Must be proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint
