What You’ll Do:
The Homecare Scheduler will be responsible for scheduling of homecare services for PACE participants.
Key Responsibilities
- Maintains homecare, holiday, on-call and weekend schedule for CENA/PCA staff.
- Maintains homecare scheduling software PACECare 365 which includes but not limited to daily/weekly. scheduling, revamping schedule, and communication with family and participants.
- Communicates with participant care manager regarding CENA/PCA pick-up and trades.
- Maintains service notes within the EMAR related to services provided to participants.
- Works with contracted agencies to secure in-home staffing as needed.
- Works with aide staffing with communication of care needs, scheduling questions or concerns.
- May assist in participant homes when staff is needed.
- Demonstrates active participation in the QI process such as chart reviews.

Skills, Knowledge & Expertise
- Prior Scheduling experience for a home based healthcare model.
- Must be able to read, write, speak and understand the English language and follow written and oral instructions.
- Must be CPR certified or willing to obtain.
- At least one (1) year of experience working with the elderly.
