Job Title/Position: Hospice Admission Nurse

Job Description Summary
We are seeking a dedicated Hospice Admission Nurse to join our team and play a pivotal role in delivering exceptional care during life's most challenging moments.
As a Hospice Admission Nurse, you will play a crucial role in providing compassionate care and support to patients and their families transitioning into hospice care. You will be responsible for assessing patients' needs, coordinating admissions, and ensuring a smooth transition into hospice services. Your focus will be on delivering high-quality, personalized care while respecting patients' dignity and autonomy during this sensitive time.
Essential Job Functions/Responsibilities
Conduct comprehensive assessments to determine patients' eligibility for hospice care in consultation with the attending physician and medical director.
Coordinate and perform admissions, ensuring all necessary documentation is completed accurately and promptly.
Educate patients and families about hospice services, treatment options, and end-of-life planning.
Develop individualized care plans based on patients' goals, preferences, and clinical assessments.
Collaborate with physicians, social workers, chaplains, and other team members to optimize patient care.
Provide emotional support and counseling to patients and families throughout the admission process.
Monitor patients' conditions and response to treatment, adjusting care plans as needed.
Ensure compliance with regulatory standards and hospice policies in patient care delivery.
Ordering and coordinating delivery of approved medications, medical supplies, and durable medical equipment to meet the needs of the patient and family.

Position Qualifications
- Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting and one (1) to two (2) years of recent experience in hospice or palliative care.
- Current Registered Nurse licensure in the State of Georgia and CPR certification.
- Excellent observation, verbal and written communication skills, critical thinking and problem-solving, basic math, and nursing skills per competency checklist.
- Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Proficiency in computer applications, including electronic medical records systems and Microsoft Office Suite.
- Ability to maintain confidentiality and adhere to ethical standards in handling sensitive information.
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
