The Clinical Care Coordinator Supervisor plays a pivotal role in guiding and developing Clinical Care Coordinators who serve youth ages 15–24 navigating complex systems. You’ll ensure program fidelity, compliance, and positive outcomes through supervision, training, consultation, and data-informed practices. Working closely with the TAY Clinical Director and leadership team, you’ll help build a resilient, adaptive team culture rooted in Wraparound with Intensive Services (WISe), MAP+WRAP, and the Golden Thread.

You’ll receive specialized training in:

• Clinical Supervision
• WISe and MAP (Managing and Adapting Practice)
• Child and Adolescent Needs and Strengths (CANS) assessments
• Portland State University’s Pathways Transition Training

Responsibilities:

• Supervise and mentor Clinical Care Coordinators, providing consultation and professional development
• Monitor staff performance and program outcomes through data and direct observation
• Lead training efforts and serve as an EHR Super-User to support documentation and compliance
• Ensure fidelity to WISe, MAP+WRAP, and the Golden Thread through case monitoring and team support
• Review and approve clinical documentation, ensuring alignment with standards and billing requirements
• Collaborate with community partners and contractors to strengthen team practices and model fidelity
• Promote a solution-focused, strength-based approach across all supervisory and clinical interactions
• Provide 24/7 on-call support for staff and cover other supervisors as needed
• Attend agency, community, and regional meetings to support program development
• Participate in continuing education and ensure staff training needs are met
• Support administrative duties for the TAY program and the Integrative Counseling Division
  

Qualifications:

• Required:
  • Master’s degree in Social Work, Psychology, Counseling, or related field (preferred)
  • OR Bachelor’s degree with 4+ years of experience working with youth with systems involvement
  • Ability to obtain Agency Affiliated Counselor (AAC) or higher DOH credential
  • Experience coordinating care and leading wraparound meetings using WISe Guiding Principles
  • Strong organizational, leadership, and interpersonal skills
  • Ability to work in community and home-based settings
  • Availability for evening/weekend work and on-call support
• Preferred:
  • Previous supervisory or leadership experience
  • Familiarity with Positive Youth Development and Evidence-Based Practices
  • Experience with electronic health records and mental health documentation
  • Knowledge of systems including Juvenile Justice, DCYF, schools, and community supports
