Daily Responsibilities:

• Coordinates care for participants and provides educational information regarding medication and disease states
• Performs assessments and data analysis of claims to determine appropriate care needed
• Develops/implements appropriate care plans based on assessment findings
• Identifies barriers and potential risks
• Provides needed social support and resources needed for care including benefits/community services

Hours for this Position:

• Monday-Friday 8am-5pm

Qualifications

Requirements:

• Must be Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
• 2+ years of professional experience as an LSW or LCSW
• 2+ years of Case Management, Community Health, or Care Coordination experience
• Must be able to travel locally (valid driver's license and auto insurance required)
