LIFE SKILLS COORDINATOR

REPORTS TO: Program Director Transitional Living

QUALIFICATIONS:

  • Must be 25 years of age or older
  • Must have a bachelor’s degree from an accredited college or university.
  • A college major in the social science field, psychology, social work, or sociology is considered a plus.
  • Experience that includes the following is considered a plus:
  • Providing support services to youth and young adults who may have experienced child abuse and neglect, are in foster care, are homeless, are at risk or have mental or behavioral health challenges.
  • Providing independent living services to youth and young adults.

PRIMARY FUNCTION: Monitor the day-to-day operations of a Supervised Independent Living (SIL) home, serving in the role of a mentor; providing guidance, oversight, and coordination of life skills development opportunities to help residents become more self-sufficient and transition them to independent living.

GENERAL RESPONSIBLITIES AND DUTIES


  • Develop and implement anage-appropriate Life Skills Transition training program for youth aged 14 through 20 who are participants in the Transitional Living program of Hope Haven of East Texas.
  • Monitor and guideparticipants of Supervised Independent Living (SIL) and the GOH with basic daily living requirements such as nutrition, maintainingscheduling, transportation, housekeeping, financial management, employment, education, personal development and spiritual foundations.
  • Assist in maintaining recordsas required by Texas DFPS contract for the SIL program.
  • Collaborate with Hope Haven personnel in facilitating Transition to Adult Living services.
  • Collaborate with Hope Haven personnel in implementing Life Skills plans for all residents aged 14-20.
  • Anticipate and maintain records necessary to track participants' progress in performance objectivesestablished for Transition to Adult Living.
  • Support the House Manager to ensure meal plans and basic food staples are maintained at the SIL.
  • Monitor and report on SIL residence compliance with Life Skills progress.
  • Consult with supervisor on specific needs of young adults and report all serious incidents immediately.
  • Serve as a mentor to young adults in the program by being physically present in the home and for an amount of time sufficientto develop a trusting relationship and by being available for the young adults to contact with questions or needs as they arise.
  • Participate in required training to maintain compliance with Minimum Standards. For this position, thirty hours of job-related training are required for the first year of assignment to child-placing responsibilities. Thereafter, twenty hours of job-related training are required.
  • Perform other assignments and duties as requested.
