Job Overview

The Linkage to Care Coordinator II is responsible for the seamless enrollment into medical care and ancillary services of all newly diagnosed HIV and Hepatitis C patients and patients transferring care to AltaMed. TheCoordinatorsupports patients to reduce barriers to care by facilitatingaccess to medications and linkage to pharmacy services, connecting clients to Behavioral Health and substance abuse services, and linking clients with appropriate case managementservices and prevention counseling.

$26.91 - $33.53 hourly
