Are you an experienced Licensed Vocational Nurse looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!

Daily Responsibilities:

• Assessing patients’ ADLs (Activities of Daily Living) and need for services that allow them to remain out of the hospital; working with the ABD (aged, blind, disabled) population

• Collaborate with medical, behavioral, and social support staff in conducting screening and risk assessment interventions based on program guidelines

Hours for this Position: Monday-Friday 7:30AM-6:00PM

Qualifications

• LTC or Home Health experience

• Experience coordinating services and knowledge of community/government-funded services in the area

• Bilingual English/Spanish highly preferred
