About Sage Health
At Sage Health, we believe every senior deserves concierge-level primary care and wellness—without the concierge fees—because they have earned it.
We operate exclusively in the Medicare Advantage space under a fully capitated, value-based care model. Unlike traditional fee-for-service medicine, our only priority is keeping patients healthy, independent, and thriving in their homes and communities.
At the heart of our model is to “EAT” at Sage:
- Engagement: We meet patients where they are and build relationships that inspire healthier lives.
- Access: We remove barriers to care through high-functioning care teams, proactive outreach, and thoughtful care design.
- Trust: We earn trust by being consistent, transparent, and exceeding expectations at every touchpoint.
We also believe exceptional patient care starts with exceptional physicians. At Sage, we intentionally hire and develop physicians who embody four essential traits:
Humble. Hungry. Human(e). Hardy.
These four characteristics define not only who we recruit, but how we lead.
The 4H Leadership Expectations
Humble: You lead with curiosity rather than ego. You seek feedback, coach through data, and create psychological safety for physicians and care teams.
Hungry: You proactively identify opportunities to improve outcomes, build systems, and elevate those around you.
Human(e):You recognize that relationships are the intervention and that exceptional medicine is rooted in trust.
Hardy: You thrive in complexity, adapt to change, and build resilient teams that endure.
