Job Summary

• Perform annual wellness visits and health assessments on a geriatric population with chronic conditions
• Provide patient with health education and teaching
• Assist in closing quality care gaps (screenings and labs)
• Team player attitude is a must and have a willingness to contribute to the continuous enhancement of care delivery workflows

Qualifications:

• Must have a active NP, or MD License for NV.
• National Certification, experience in primary care/adult/geriatric, EMR skills preferred
• Position is an Independent Contractor, and it is based on per visit.

Job Locations:

• Reno, NV

Preferred experience:

• Previous experience with In-home risk Assessment.
