What will your responsibilities be?
- Undertake functional assessments to promote independence and wellbeing
- Support transformation and improvement initiatives within adult services
- Complete Care Act–compliant assessments and reviews
- Recommend equipment, adaptations, and reablement solutions
- Work collaboratively with internal teams and partner agencies
- Contribute to efficient discharge planning and prevention of unnecessary care packages

Essential Requirements:
- Degree or Diploma in Occupational Therapy
- HCPC registration
- Experience working within Adult Social Care
- Full UK driving licence
- Access to a vehicle
- Willingness to travel both inside and outside the council area as required
