Job Overview & Work Site

Your Patients You’ll work with a diverse caseload, including a higher‑than‑average post‑operative orthopedic population. How You’re Supported You’re set up to do your best work through: • Thoughtful scheduling and defined territories to support appropriate caseloads • Limited drive time between visits whenever possible • Clinical leadership from therapists who understand day‑to‑day practice demands • Individualized support aligned with your clinical interests and goals • Collaboration with Home and Heart healthcare colleagues for a team‑based approach to care

License & Experience

Requirements for this position include the following:

• Graduate (current or pending) from an accredited physical therapy program
• Active or pending state physical therapist license
• Recent graduates encouraged to apply

Annual Incentive Bonus Up To
