Seeking an Experienced Psychiatric Nurse Practitioner to work as part of the primary care treatment team at RI Health center
Practitioner identifies, triages, and manages patient care for behavioral health patients within the primary care setting.
Oversees the progress of a specific caseload of patients across the care continuum including performance of psychiatric assessments, patient examinations, review of relevant medical records and collection of historical data from family and healthcare providers.
Work with medical staff regarding clinical status and treatment recommendations. Provide appropriate education, consultation, and resources for psychiatric patients across the care continuum.
The practitioner also will employ patient education strategies to develop specific behavioral change plans for patients and behavioral health protocols for target populations.
The Practitioner will coordinate prescription refills and provide follow up as needed.
