The facility will not be able to offer this position to any candidate who use tobacco or nicotine. This also includes the patch or any other devise/product that contains nicotine. Please be sure to only submit candidates that meet this requirement.

Facility will only consider Pt's with at least 1 year of home health experience (within the last three years).
