JOB TITLE:
- Qualified Mental Health Professional
OVERVIEW:
The Qualified Mental Health Professional provides mental health case management services to children, adolescents, adults and their families, and completes documentation for service coordination, screenings, aftercare, skills training, and plans of care.
RESPONSIBILITIES:
- Assessing clients' mental wellness, needs, preferences and abilities, and developing plans to improve
- Working with clients, family and friend support networks, and health care professionals to put care plans in place
- Listening to clients' concerns and providing counseling or intervention as required
- Recording clients' progress, charting referrals and scheduling home visits
- Evaluating clients' progress periodically and making adjustments as needed
- Following up with discharged clients to ensure they are satisfied with services and still in good physical and mental health

- Follow service implementation processes designed to meet the needs of people served and funding-source requirements.
- Emergency/Crisis Intervention skills.
- Ability to proficiently work with computers and electronic records.
TRAVEL:
- Field-based assignments require travel within the counties that we serve in this area (Hamilton, Butler, Clermont.)
Hours
Between 9am and 9pm Monday-Sunday
Salary
15.00 per hour plus mileage
Requirements:
High School Diploma/GED
Clean FBI/BCI check
Reliable transportation and insurance
COVID-19 precautions
