Position Title: Agency Nurse
Horizons creates and sustains person-centered opportunities for people with special needs where they live, learn, work, and play. The Agency Nurse is to be responsible for ensuring the physical and emotional health and safety of each participant in Horizons Programs Inc. The Agency Nurse can facilitate requested trainings for agency staff as needed.
In this position you will:
- Conduct DDS Medication Administration Certification classes and ongoing recertification classes.
- Participates in ongoing training required by the Department of Developmental Services, Horizons Policies and Procedures, Employee professional goals and to meet the changing needs of the program.
- Converse with participants to reinforce positive behaviors and to promote socially appropriate interaction with peers and others.
- Consult with the Directors/Managers and carry out assigned duties.
- Conduct other health related trainings for various parts of the agency or specific program trainings.
- Have the best interests of each participant as the basis of your daily performance.
- Act as a link to community, emergency, medical, recreational and service providers.
- Enthusiastically assist in carrying out the Individual Plans for each resident which includes instruction in independent living skills, social skill development, recreation, and community integration.
- Ensure programs are in compliance with all DDS Licensing and Medication regulations.
- Follow the policies and procedures of Horizons, Inc. and Horizons Programs, Inc. including but not limited to being a mandated reporter of suspected abuse and neglect.
- Attend and actively participate in designated in-service trainings and requested staff meetings.
- Communicate with participants in a manner they understand, such as total communication and sign language
- Administer medication as directed by the Manager in accordance with Physicians orders and DDS Medication Administration guidelines.

Requirements:
- At least one year minimum experience working successfully in the medical field.
- At least one year experience working with people who are developmentally disabled, have autism, and exhibit challenging behavior is required or an approved equivalent.
- Be at least 21 years of age, have an Associate Degree in Nursing or a Bachelor of Science degree in Nursing from an accredited school as well as a current, valid Connecticut Nursing License.
- Must possess a valid Connecticut driver's license and acceptable driving record.
- Certification in CPR is preferred or must be successfully completed within the first 30 days of employment (Horizons offers these courses to employees).
- Certification in First Aid is preferred or must be successfully completed within the first six months of employment (Horizons offers this course to employees - Waived for LPN and RN).
- File application, a letter of interest, at least 2 work references, 1 personal reference and current health form prior to working.
- An acceptable background check and nursing license check performed by Horizons.
