Assesses the resident’s condition on an ongoing basis utilizing the nursing process. Documents information gathered in accordance with the nursing process and facility policy and procedures.

Evaluated resident’s condition and identifies the need to notify the attending physician of any change in the resident’s condition, etc.

Administers medication and treatment and provides direct resident care.

Participates in the care planning process in coordinating nursing services with other discipline services.

Directly responsible for supervision of the daily work assignments and performances of assigned activities of nursing staff (i.e. LPN’s, MA’s, C.N.A’s) in accordance with the Nurse Practice Act.

Prepares and implements employee performance evaluations for LPN’s, MA’s, C.N.A.’s.

May assume charge nurse duties to meet facility needs.

Participates in the Quality Assurance Program.

May perform other varied duties/responsibilities as assigned per facility needs. Team leadership is a must in this role. We are seeking a strong individual who can lead and supervise the night shift.

Qualifications

Graduate of Accredited/Licensed School of Nursing.

Licensed to practice as a Registered Nurse in the State of Wisconsin as defined by S. 441.11(1) Wisconsin Statutes.

Previous work experience in geriatrics, medical surgical, psychiatric or the Developmentally disabled preferred.

Understands and effectively carries out oral and written instructions.

Understands the concept of comprehensive nursing care.

Ability to plan and direct the work of others.
