About this role
As a PRN Registered Nurse for Prime Home Health, you will provide compassionate and skilled nursing care to adult patients in the comfort of their homes. We have many visits available in all areas of San Antonio and other surrounding towns. Prime Home Health can provide you with competitive rates for visits and OASIS tasks along with paid mileage with an experienced, supportive administrative team.
You will be responsible for assessing patients' needs, developing care plans, coordinating with other healthcare professionals, and implementing appropriate interventions to promote optimal health and well-being. Your expertise, clinical judgment, and strong interpersonal skills will be crucial in delivering high-quality care and ensuring positive patient outcomes.
Responsibilities:
- Patient Assessment and Care Planning:
- Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop individualized care plans.
- Collaborate with patients, their families, and healthcare teams to establish goals and interventions to address patients' health conditions and promote self-management.
- Regularly reassess patients' conditions, monitor vital signs, and evaluate the effectiveness of interventions, adjusting care plans as necessary.
- Educate patients and their families on disease management, medication administration, and self-care techniques.
- Skilled Nursing Care:
- Provide skilled nursing care including wound care, medication management, IV therapy, catheter care, and other treatments as prescribed.
- Administer medications and treatments accurately and safely, following established protocols and guidelines.
- Perform skilled procedures such as venipuncture, tracheostomy care, and respiratory therapy.
- Monitor and manage symptoms, pain, and comfort levels, implementing appropriate interventions to alleviate distress.
- Coordination and Collaboration:
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and implement comprehensive care plans.
- Coordinate with the interdisciplinary team to ensure continuity of care and effective communication.
- Communicate regularly with patients, families, and caregivers to provide updates on the patient's condition, progress, and any changes to the care plan.
- Coordinate and supervise home health aides and other support staff involved in patient care.
- Documentation and Compliance:
- Maintain timely, accurate and thorough documentation of assessments, care provided, and patient outcomes in Kinnser/Wellsky (EHR).
- Ensure compliance with all regulatory requirements, agency policies, and standards of care.
- Complete and submit clinical documentation, including OASIS assessments, within specified timeframes.
- Quality Improvement:
- Participate in quality improvement activities, including case conferences, chart reviews, and performance improvement initiatives.
- Identify areas for improvement in care delivery and actively contribute to the development and implementation of best practices.
- Stay updated with the latest evidence-based practices and industry trends related to home health nursing.
Requirements:
- Current and unrestricted Registered Nurse (RN) license valid in the State of Texas.
- Minimum of one year of nursing experience in adult home health.
- Experienced in all OASIS tasks: SOC, ROC, Recertification, Discharge and regular visits.
- Experience with Kinnser/Wellsky a plus, additional training on Kinnser can be provided by our team.
- Strong clinical assessment and critical thinking skills to effectively address patients' complex needs.
- Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.
- Ability to work autonomously, prioritize tasks, and manage time efficiently in a dynamic home health environment.
- Willingness to travel to patients' homes within the designated service area and have reliable transportation.
- BLS (Basic Life Support) certification.
