Registered Nurse (RN) - Hospice & Home Health

Key Responsibilities

• Conduct comprehensive head-to-toe patient assessments and build customized care plans tailored to individual goals.
• Perform wound care, IV therapy, symptom management, and medication reconciliation in the field.
• Serve as the primary clinical liaison, coordinating directly with physicians, families, and our interdisciplinary team.
• Provide vital patient and caregiver education on disease progression and emotional guidance through complex care journeys.
• Utilize EMR systems to ensure timely, accurate charting that meets regulatory compliance.

Qualifications

• Current Texas RN license and BLS/CPR certification.
• 1+ years of clinical nursing experience (Hospice preferred).
• Reliable vehicle, valid TX driver’s license, and auto insurance.
• Bilingual (English/Spanish) is a major plus.
