Candidates will be responsible for performing Case Managements duties including:

• Assessments
• Develop and implement care plans
• Track member progress
• Hospitalization Review
• Follow-up with providers
• Care Coordination

Hours for this Position: Monday-Friday (40 hours/week)

Pay: $30 per hour

Qualifications

• Clear and active RN license in the state of MO
• Strong clinical background
