Are you an experienced Registered Nurse with Discharge Planning experience looking for a new opportunity with a prestigious healthcare company? Job Description:

• Concurrent review nurses are concerned with transition of care and making sure patient care is being provided in the most cost efficient way possible (getting patients out of the hospital and into a less costly but still safe environment). On this particular team, they’ll monitor the care of a patient from the inpatient phase through the post-acute phase of care
• This “Transitional Care Unit” is a newly created department in the company. Nurses in this unit will deal with Medicaid cases, so the patients’ ages would vary from younger to older. It’s a team of about 10.

Qualifications

• Active RN license
• Utilization Review or Discharge Planning experience
• Inpatient hospital experience (MED-SURG is ideal)
• Computer skills! Some specific software they’ll be using: TrueCare, Word & Outlook (email & calendar)

Additional Information

Pay Range:

$32-36/hr (Based on Experience)

Shift:

Shift Mon-Fri 8:00-5:00
