We are currently seeking an experienced and compassionate RN Nurse Care Coordinator to join our dedicated team at a long-term care nursing facility in Staten Island, NY.
This is an exciting opportunity for a Registered Nurse who thrives in a leadership and coordination role while making a meaningful impact in the lives of residents and their families.
As the RN Nurse Care Coordinator, you will play a key role in overseeing resident care coordination, interdisciplinary communication, discharge planning, and family support while helping maintain the highest standards of care throughout the facility.

Key Responsibilities
- Coordinate and oversee resident care plans alongside interdisciplinary healthcare teams
- Communicate with physicians, nurses, therapists, residents, and families regarding care updates and treatment plans
- Assist with admissions, transfers, discharge planning, and care transitions
- Monitor resident progress and ensure care plans are being followed appropriately
- Advocate for residents’ healthcare needs and overall well-being
- Ensure compliance with nursing home policies, state regulations, and quality care standards
- Participate in care conferences and collaborate with department heads
- Maintain accurate documentation and resident records
- Help support and educate residents and family members regarding care services and treatment plans
Requirements
Requirements
- Active RN License in New York State
- Previous experience working in a nursing home or long-term care facility preferred
- Experience in care coordination, case management, MDS, admissions, or supervisory nursing roles is strongly preferred
- Must be fluent in Filipino and English
- Strong organizational and communication skills
- Ability to multitask and work effectively in a fast-paced healthcare environment
- Compassionate, professional, and resident-focused attitude
- Reliable and team-oriented
