Job Summary:
The Speech-Language Pathologist (SLP) provides skilled speech and language therapy services to adult and geriatric patients in their homes. The SLP evaluates, diagnoses, and treats speech, language, cognitive-communication, voice, and swallowing disorders to promote optimal recovery, independence, and quality of life.
Key Responsibilities:
- Perform initial and ongoing speech therapy evaluations in the patient's home.
- Develop and implement individualized care plans based on patient assessment and physician’s orders.
- Provide therapeutic techniques for the rehabilitation of patients with speech, language, voice, cognitive, and/or swallowing disorders.
- Instruct patients, families, and caregivers in therapy techniques and strategies.
- Collaborate with interdisciplinary team members (nurses, occupational therapists, physical therapists, etc.).
- Maintain accurate and timely documentation in accordance with state and federal regulations, payer requirements, and company policies.
- Educate patients and families about goals and progress.
- Reassess and adjust care plans as necessary.
- Adhere to infection control and safety policies during home visits.
Qualifications:
- Current, active license as a Speech-Language Pathologist in the state of Florida .
- Certificate of Clinical Competence (CCC-SLP) preferred
- Minimum of 1 year experience in a clinical or home health setting (preferred).
- CPR certification (required)..
