About this role

The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.


The North Lambton Community Health Centre is currently seeking applicants for the following position:



Fulltime Case Manager – 1 Year Contract

Harm Reduction and Hepatitis C Care Team

37.5 Hours/Week



The Case Manager provides client-centred case management, education, advocacy and practical support to individuals living with or at risk of Hepatitis C. Working as part of an interdisciplinary team, the Case Manager conducts outreach, identifies needs and barriers to care, and helps clients navigate testing, treatment and follow-up. The role also connects clients with health, social and community services and provides education, advocacy and psychosocial support to clients and their identified supports.



Qualifications:

  • Diploma or degree in social service work, social work, addictions and mental health, community services or another related field; or an equivalent combination of education and relevant case-management experience.;
  • Minimum three years of relevant experience in community-based case management, outreach, healthcare, social services or a related setting; experience in community health or primary care is an asset.
  • Demonstrated experience developing client-centred support plans and coordinating services for people with complex needs.
  • Comprehensive understanding of harm reduction, trauma-informed practice and the social determinants of health.
  • Commitment to culturally safe, respectful and inclusive practice with diverse communities.
  • Ability to maintain timely, complete and accurate records in accordance with NLCHC policies, privacy requirements and applicable standards.
  • Strong computer skills, including Microsoft 365 and electronic medical records.
  • Knowledge of hepatitis C prevention, testing, treatment and follow-up is an asset.

This position enjoys a supportive environment with flexible hours, 37.5 hours per week Monday to Friday. The incumbent will primarily be required to work at our Forest site and outreach locations.
