Under general supervision, coordinates the intake and eligibility process. Assists the individual using services in obtaining services. Performs functional assessments and social histories. Convenes an Interdisciplinary Team. Facilitates the development of the Individual Service Plan. Establishes meaningful goals. Coordinates and monitors services in compliance with Administrative Rules, Services Management Plan, and other applicable policies and procedures. Measures progress toward goals. Maintains case files, correspondence and all other required data. Mediates conflicts and advocates for individuals using services.

Knowledge of:

oIntensive In-Home policies and procedures

oClinical diagnosis and treatments

oHuman services and MH/DD/SAS network

oPrinciples of choice, empowerment, and community integration

oDisability related income sources

oState Services and Definitions as outlined in MH/DD/SAS

oDiagnostic and statistical manual

oBasic office practices and procedures including record keeping and filing systems

oEnglish language to include grammar, spelling, sentence structure, and vocabulary; and

oProofreading skills

oGeneral math aptitude

Ability to:

SPECIFIC DUTIES:

oPerforms intake duties, and explains agency services to applicants.
oProcesses applications for services by conducting intake interviews, completing social histories, obtaining necessary assessments and determining eligibility.

oDetermines legal settlement by verifying information, contacting providers, county and family members

oIdentifies alternative funding and assists the individual using services in accessing other financial supports.

oProvides information regarding available community resources and its natural supports.

oConvenes Admission Team and coordinates development of Individual Service Plan.

oAssists individuals, their families, and their guardians in maximizing their abilities for sel1 direction by empowering them to achieve the greatest possible degrees of independence.

oMediates conflicts between providers, family members, individuals using services, legal representatives, and/or the agency relative to the needs of the individual, as identified in the Individual Service Plan.

oWorks with the Interdisciplinary Team to identify needs and establish meaningful goals

oRequests funding appropriate to identified needs.

oCoordinates the implementation and monitoring of the Individual Service Plan.

oMonitors progress toward goals using required quarterly reports

oInitiates discharge planning process to transfer/close cases when services are no longer needed or desired.

oCoordinates IIH services to established State Definitions

WORKING HOURS:

Monday through Friday, 8:30am - 5:00pm, with flexibility required to meet consumer needs. Some travel is required.

MINIMUM EDUCATION. TRAINING AND EXPERIENCE:

A graduate of a college or university with a Masters degree in a Psychology or Social Work field who holds a license issued by the governing board regulating a human service profession in NC. One year of documented experience working with the mental health population.
