Location: Los Angeles & Orange County, CA

Position Type: Per Diem / On-Call / Part Time

Compensation: Starting at $70/hour (varies by tier)

Position Overview Are you looking for a rewarding nursing career that offers independence, flexibility, and premium patient care? TheKey Concierge Nursing Services is seeking an adaptable Home Health RN to serve clients across Los Angeles and Orange County.

Currently, primary opportunities involve in-person home visits, initial assessments, and quality assurance re-evaluations, with shift-based work expanding in the near future. As a Home Health nurse, you will be called on an as-needed basis to accept assignments, with strong priority given to clinicians open to accepting last-minute requests.

Responsibilities

Key Responsibilities

• Comprehensive Assessments: Conduct in-person initial nursing assessments, reassessments, change-of-condition evaluations, and home safety reviews to build tailored, high-touch Care Plans.

• Direct Patient Care: Perform skilled nursing visits, routine wellness checks, and preventative care in accordance with physician orders and individual care plans.

• On-Demand & Flexibility: Accept per diem visit assignments on an as-needed basis, including responsiveness to last-minute patient requests or coverage needs.

• Client & Family Engagement: Serve as the main clinical point of contact, building strong rapport with clients, family members, and multidisciplinary care teams.

• Clinical Documentation & Coordination: Complete and submit accurate clinical notes in a timely manner via our electronic system, ensuring strict compliance with state home health regulations.

• Future Coverage: Transition seamlessly into block shift-work opportunities as case volumes and care needs expand in the region.

Qualifications

Qualifications

• License: Active, unencumbered Registered Nurse (RN) license in California (BSN preferred).

• Experience: Minimum 2 years of nursing experience; background in home health, elder care, or care management strongly preferred.

• Availability: Flexible schedule with willingness to work on-call, per diem, and accept last-minute visit assignments.

• Transportation: Valid California Driver’s License, reliable vehicle, and active auto insurance.

• Skills: Strong clinical assessment capabilities, exceptional customer service, direct conflict resolution skills, and tech-savviness for timely EHR documentation.
