POSITION SUMMARY

The Ilanka Community Health Center RN Case Manager works collaboratively with all members of the medical and behavioral health team to ensure access to appropriate, integrated care for the patients and clients of ICHC.

ESSENTIAL DUTIES

·Supervises Referral Coordinator position and oversees referral operations.

·Analyzes data to identify high risk and high-need patient populations for targeted case management services.

·Facilitate treatment plan changes, in consultation with medical providers, for patients requiring more intensive or specialized healthcare services.

·Assists patients in accessing necessary care resources, including the procurement of durable medical equipment and medications, particularly when resources are limited.

·Responsible for the continuity of patient care throughout the PCMH environment; serving as the liaison between Behavioral Health and Clinical departments, including Medications for Assisted Treatment (MAT) program.

·Coordinates and facilitates case management coordination meetings.

·Applies for and manages all aspects of Special Diabetes Program for Indians and any other specialized care management funding opportunities.

·Coordinate Narcan education, distribution and community outreach.

·Assists with patient triaging.

·As needed, provide direct patient care as registered nurse which may include Saturday coverage.

·Maintain competencies and participate in CLIA waived testing and associated activities as needed.

·Collaborate with the appropriate Native Village of Eyak departments in response to Tribal Member Social Determinants of Health needs.

·Serves as initial point of contact for patient Video teleconferences.

·Participates in Quality Assurance, Quality Improvement and Risk Management program.

·Maintain a high level of confidentiality in accordance with HIPAA and HITECH regulations, which includes only accessing appropriate information needed to perform job duties.

ESSENTIAL DUTIES OF ALL STAFF

·Immediately reports unsafe conditions, hazards, or injuries to HR or the designated safety contact.

·Assists with special events, meetings, and trainings as necessary.
