Senior Patient Care Coordinator

Role Overview:

We are seeking a dedicated Senior Patient Care Coordinator to join our reputable healthcare team. This position plays a crucial role in the coordination and delivery of patient care, ensuring that our patients receive the highest quality of services.

Key Responsibilities:

• Coordinate with healthcare professionals to assess, plan, and implement patient care plans.
• Monitor and evaluate patient progress, adjusting plans as necessary.
• Manage patient records and ensure compliance with healthcare regulations.
• Educate patients and their families on treatment plans and care procedures.

Required Skills and Experience:

Must have a current and valid Registered Nurse (RN) license. A minimum of 5 years of nursing experience, preferably in a supervisory or coordinative role. Experience with electronic medical records.

Preferred / Nice-to-Have Skills:

Palliative care experience and a background in patient advocacy are advantageous.

Education and Certifications:

Bachelor's degree in Nursing (BSN) is required; a Master’s degree in Nursing or Healthcare Administration is preferred.

Soft Skills and Behavioral Expectations:
