As a Support Facilitator at Kaikaranga, you will be the connection between people with disabilities and the support, services and opportunities that help them live with real choice, freedom and control.
What you will be doing
- Building genuine, trusted relationships with clients, whānau and providers
- Conducting in-depth needs assessments for people with disabilities, who have a broad range of circumstances
- Coordinating tailored support plans that reflect each person's goals
- Navigating services and opportunities to create lasting, meaningful outcomes
- Showing up differently every day - because no two clients are the same
What you will bring
- Empathy and emotional intelligence - you lead with people, always
- Strong communication and relationship-building skills
- Self-drive, initiative and a genuine desire to make a difference
- Experience in health, disability, aged care or community services (or the hunger to learn)
- Good computer literacy and confidence using Microsoft Office
- Proven experience coordinating natural and formal supports and services for people with a range of physical, intellectual, and or sensory needs
