As a Connector Case Manager at Kaikaranga, you'll walk alongside clients with high and complex needs, and their whānau, building the relationships, networks and support structures that create real, lasting change. Connection isn't just part of the job here. It's the whole point.
What you will be doing
- Walking the journey with clients and whānau to achieve meaningful, personalised outcomes
- Assessing and responding to risk, exploring options and connecting people to services and community
- Facilitate support needs assessments / reassessments according to NASC guidelines and process
- Building and negotiating short, medium and long-term support plans that reflect real goals
- Collaborating with providers and other sectors so everyone is working toward the same outcome
- Growing your networks in the community and with other professionals to broaden what's possible for your clients
What you will bring
- At least three years' experience in health, allied health, education or disability
- Proven experience coordinating natural and formal supports for people with physical, intellectual or sensory needs
- Working knowledge of TeTiriti o Waitangi and the ability to coach others in its practical application
- Commercial savvy and confidence navigating operational IT systems
- The kind of communicator who builds trust quickly across a diverse range of people
